The Louisville Public Library, in partnership with the Colorado Department of Public Health & Environment (CPDHE), will be hosting a COVID-19 Vaccine Clinic at the Louisville Public Library (951 Spruce Street).
Appointments are required for both doses, please use both links below to register. Please only register if you can attend both clinics. We will be unable to accommodate requests from individuals who miss the scheduled 2nd dose clinic.
Friday, November 12 - First dose Pfizer/Pediatric Pfizer clinic
Friday, December 3 - Second dose Pfizer/Pediatric Pfizer clinic
No ID or proof of insurance is required. The vaccine is available free of charge. To find out more about the COVID-19 vaccine, visit the CDPHE website.
After receiving your vaccine, you will receive a vaccination record card. This is an important document to keep for your records as it informs you of your second vaccination time and date. You will then be asked to park or sit and wait in a designated area for a 15-minute observation period. If you are not feeling well during this observation period, an on-site health professional will assist you.
Please remember to stay hydrated before and after your vaccination and please eat before your vaccination or bring a snack with you. Also, feel free to bring a friend or family member with you for support if needed.
Masks are required regardless of vaccination status.
