The Conversation Project-Advance Care Planning

Advance care planning is the process that allows people to make plans about future health care. Who would speak for you or make decisions for you if you couldn’t for yourself? About 50% of us will be unable to make decisions at some point in our lives. Join The Conversation Project in Boulder County to learn about advance care planning and advance directive documents. Most importantly, we will help you start talking about healthcare values and wishes with the people that matter most. You will leave this session with useful tools and steps you can take to start the advance care planning process. Presented through the Zoom platform. Zoom link will be shared via email to those registered.
